Iowa law requires all students present a valid immunization record before they can enroll in school. Proof of immunizations must include submission of one of the following immunization records:
- Iowa HHS completed immunization record,
- Iowa HHS religious exemption record or
- Iowa HHS medical exemption record signed by a prescribing doctor specifying that the immunizations required would be injurious to the student or to any household member.
- Please note: Exemptions do not apply when the Iowa Council of Health and Human Services has determined and the Director of Iowa Health and Human Services has declared an emergency or epidemic exists.
Immunization forms are available in your school health office, on the District website, at the Buchanan County Health Department, or through your child’s health care provider.
Please contact your school nurse with any questions.
When possible, medications should be scheduled so that it is taken at home and not during school hours. Students are not to carry medication or keep it in their possession except inhalers for asthma or auto-injectors of epinephrine for severe allergies. Medication may only be given by the school nurse or school personnel who have completed a medication administration course. By law, school nurses are not allowed to administer homeopathic or non-FDA-approved medication.
If medication is required to be administered at school, the following protocol applies:
- Parents must complete a Medication Authorization Form
- Medications must come to school in the original, labeled container as dispensed or the manufacturer’s labeled container
- The medication label must contain the student’s name, name of the medication, directions for use, and date
- Authorization is renewed annually and immediately when the parent notifies the school that changes are necessary
- Pharmacies will provide, at no cost, a second labeled bottle for school use
